Флаг Пий-Хемского кожууна представляет собой прямоугольное полотнище: левая часть, вдоль древка — жёлтого цвета; правая часть — голубого цвета с полосой, обрамленной белыми полосами, проходящей по диагонали снизу вверх от нижнего края жёлтой части. Цвета флага символизируют голубую тайгу, реку «Бии-Хем» приток Енисей; жёлтый — традиционный цвет буддийской религии.

Соотношение ширины к длине флага: 2:3.

Ширина полосы жёлтого цвета — 1:5 длины.

Ширина голубой диагональной полосы — 1:20 ширины.

Ширина белой окантовки диагональной полосы — 1:20 ширины.
